Job Description
The Customer Service Representative/Patient Care Coordinator position is responsible for coordinating all aspects of home health care for the patients we serve. The position requires a detail-oriented person with data entry/computer experience and the ability to effectively communicate both orally and in writing with referral agents, patients, and clinical staff. Data entry with accuracy is necessary with extended time at a computer and on the phone. Physical activity is sedentary and may require occasional lifting or carrying up to 10 lbs.
Duties:
o Staff cases with qualified personnel with a sense of urgency and organize/reorganize job schedules for home care personnel to ensure that all clients receive the appropriate care.
o Process all new admissions, re-certifications, and resumptions by entering the Plan of Care and ensuring that all required documents have been submitted by the nurse/therapist.
o Plot schedules according to the Plan of Care and communicate them to clients and field staff. Distribute schedules to workers on a weekly basis.
o Ensure that all SN, HHA, PT/OT/ST, and MSW frequencies are according to the Plan of Care.
o Work closely with the Intake Coordinator to ensure that all patients have adequate authorization.
o Retrieve messages from the Clinical Report Line periodically throughout each day.
o Receive new orders via fax and enter them into the computer with accuracy on a daily basis.
o Receive lab results via fax and immediately distribute them to the nurse, physician, infusion pharmacy, and patient chart.
o Process all supply orders and DME requests based upon the patient's insurance/payer source.
o Remain in communication with clients and field staff to ensure the quality we promise is delivered.
o Ensures that supervisory visits are made within the appropriate time frame, including 13/19/30 day re-assessments for Medicare patients receiving therapy.
o Perform on-call coordinator duties as needed.
o Ensure compliance with all federal, state, and local government laws and regulations.
o Completes other assignments as requested and assigned.Job Requirements
o High School Graduate with BA/BS degree preferred.
o Minimum of one (1) year work experience in a healthcare setting or related scheduling field required. (Prior home health experience and/or scheduling experience preferred).
o Team player and excellent customer service skills. Must be detail-oriented and have strong organizational skills. Ability to multi-task, works under pressure with changing priorities and short deadlines, and effectively handles a heavy workload.
o A good working knowledge of computer skills, including, but not limited to Microsoft Office.
