Summary:
Wellness/Health Coaches assist patients in realizing their potential for change through:
motivational interviewing, goal setting, the dissemination of education and information, and increased accountability. Wellness Coaches empower participants and their families by providing goal driven, well-developed plans for them to get and stay healthy. They also assist in tracking patients' progress and providing positive reinforcement for their efforts. The Wellness/Health Coach also develops and conducts onsite health-related seminars and training courses for clients and employee groups. This position works mostly telephonically, entering notes and comments into an electronic coaching platform program while conversing with the client.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Wellness/Health
Provides one-on-one telephonic coaching to assigned individuals seeking to achieve improved health and well-being.
Provides individualized behavior change coaching to reduce or eliminate high-risk behaviors.
Reviews pertinent medical history, current diagnosis and pharmaceutical data from the electronic medical record (EMR) and utilizes that information when suggesting effective goals and motivational steps with the patient.
Intervenes effectively with patients who are in emotional crisis, including suicidal.
Effectively discerns appropriate Scope of Practice for the coaching role and discusses any concerns with the immediate supervisor.
Applies evidence-based strategies and guidance in telephonic coaching sessions for adoption of specific behaviors shown effective by research to accomplish improved health and health maintenance.
Assesses patients referred for Medical Nutrition Therapy and plans appropriate response and information.
Works with patients referred for Medical Nutrition Therapy to develop prescriptive plans of action.
Transitions patients who have completed Medical Nutrition Therapy back into the coaching process.
Assists individuals in tailoring behavior goals and action steps specific to their personal needs and priorities.
Develops customized care plans for participants including goals and a documented plan of action.
Uses effective engagement skills and processes to retain participants and ensure a high level of participant satisfaction.
Effectively utilizes the coaching application to manage member caseloads.
Uses the coaching application, including scheduling, preparing for, and documenting coaching sessions for accurate reporting.
Participates in ongoing quality assurance and training meetings as required.
Coordinates with other team members and associated staff to ensure participants access the services and programs they need to optimize their well-being.
Assesses and identifies areas of interest and significance with the patient and directs goals to those identified areas.
Effectively utilizes readiness to change in goal setting, guide to care, productivity measures and on-going assessment.
Identifies, develops and provides health education materials to employees and family members.
Coordinates care with other available health-related specialists and coaches.
General
Reinforces organizational core values and behavioral expectations through modeling, communication, and coaching.
Maintains a clean, neat work environment and presents a professional image at all times.
Treats all CareHere employees, customers, clients, vendors, and patients with courtesy, respect and dignity.
Maintains strict confidentiality related to company, client, patient, and employee information.
Understands and follows safety and security procedures and practices preventative safety procedures as set forth by CareHere.
Immediately reports all accidents and incidents to the immediate supervisor or Human Resources.
Uses only equipment trained to use and operates all equipment in a safe manner.
Travels by car, air or other means as necessary to conduct company business (approximately 25%).
Fully understands and adheres to the systems provided by CareHere to control assets and expenditures.
Adheres to all company policies, procedures and business ethics codes.
Executes all terms and conditions as set forth in the CareHere Employee Handbook.
QUALIFICATIONS
Bachelor's degree or higher in a health/wellness related discipline
Completion of Dietetic Internship with a Registered Dietitian Certification
Familiarity with and ability to apply Medical Nutrition Therapy as prescribed
Two (2) years' experience in a position related to nutrition
Wellness/Health Coach certification from a nationally recognized and accredited professional wellness coach training program.Preferred certifications:
National Society of Health Coaches (NSHC), Dr. Sears Wellness Institute, The L.E.A.N. Group, American Council on Exercise (ACE), Duke Integrative Medicine,
Wellcoaches School of Coaching and American College of Sports Medicine (ACSM)
Knowledge of specific disease and lifestyle related topics such as:
smoking cessation, weight management, nutrition, exercise, stress reduction and chronic conditions required
Outstanding interpersonal and communication skills required, specifically, the ability to engage in a dynamic clear conversation over the phone
Excellent keyboard and internet navigation skills, including the ability to use online applications to document accurately and concisely, while simultaneously talking on the phone
Flexibility to work with variable individual schedules across multiple time zones
Demonstrated commitment to continuing professional development; open to learning new and innovative ways of delivering behavioral lifestyle intervention services and dedication to continued development of behavioral coaching skills
Ability to be self-directed, highly organized, think critically and be proactive
Computer literacy of Microsoft Office products and email skills
